These forms are for practitioners and employers who want to apply for a licence to administer radioactive substances to people in diagnosis, therapy or research. There is a fee for employer applications. Details of the fees are available online.

Further advice on making an application is available for practitioners and employers on the website and in theARSACnotes for guidance.

All employer and practitioner licence applications must be submitted through theonline portal.

Particular patient licence applications should be submitted by email to arsac@ukhsa.gov.uk

If you are a research sponsor, please read our information onhow to apply forARSACapproval for a research study.

Please note that applications submitted on the incorrect form will be returned.
