Fill in form EC3164 and send it to the address on the form.
You also need to send a plan showing the whole of the premises to be approved.
To get approval, you must have your holding inspected by an Animal and Plant Health Agency (APHA) vet.
Find a list of animals covered by the Balai Directive.
