• that youve moved to a new practice, if youre an OV or ATT
• about a change to a practices details, such as name or address

Once your form has been processed, APHAs OV team will update all relevant details on Sam, APHAs online system for recording tuberculosis (TB) test results.

APHA uses these details to send you important information about your OV authorisation. Its your responsibility to make sure the details are correct.
