What this form is for

This form is for claiming back the legal costs you paid as part of your infected blood interim estates payment application. This means:

• The exact cost of your legal fees towards the cost of applying for a grant of probate or confirmation up to a maximum of 1,500 and/or;
• The court fee for obtaining a grant of probate, confirmation or letters of administration (exact amount).

You can only claim for legal costs if your claim for compensation is successful. Check your eligibility and find out more information.

Who should complete this form

To complete this form you must be the person who has paid for legal support to obtain probate, confirmation or letters of administration.

Before completing this form

To fill out this form, you will need your:

• Application Reference Number for the interim payment, which you can find on your compensation application decision letter.
• The details of the legal representative you used.
• Receipt of payment of your court fee to obtain probate (England, Wales and Northern Ireland) confirmation (Scotland) or letters of administration.
• Proof of payment for legal services, such as an invoice from your legal representative
• Confirmation of payment.

After completing this form

Once you have completed a successful application you should send your completed form and copies of relevant invoices to the Infected Blood Support Scheme who processed your claim for compensation.
