From 1 November 2024, a new online service to register with a GP surgery will replace the GMS1 form. There will be anew paper registration form (PRF1)for patients who are not able to register online.
GP practices must register for the new service by 31 October 2024.

Complete the GMS1 form to register with a GP.
Print and fill in the form and return it to the practice you want to register with. The form cannot be completed online.
