If there is a mistake on your Disclosure and Barring Service (DBS) certificate, the dispute and fingerprint consent forms and guidance explain what to do to raise a dispute with DBS, and what you need to do if you have to provide fingerprints.
The form should only be used for Standard and Enhanced certificate dispute, and cannot be used to raise a dispute relating to a Basic certificate.

Information has been added to section D of the form, regarding a time limit for raising a dispute.
Certificate dispute form and fingerprint consent form have been combined into one form, and the guidance has been updated in line with this.
