date and time of your theory test
address of the theory test centre

Youll need your UK driving licence number.

You do not need to take your booking confirmation to your theory test.

Contact the Driver and Vehicle Standards Agency (DVSA) to get help checking your appointment details.

If you want to change or cancel your theory test by phone or email, youll need to give your:

full name (as it appears on your driving licence)
date of birth
address, including your postcode
driving licence number
booking reference number
