Submitting your application
The MSF5100 application form is for owners and operators of ships and fishing vessels to request a survey or inspection.
The form also contains information about health and safety standards and the arrangements you must make to make sure the surveyor can inspect the vessel safely.
It includes guidelines on:
safe means of access
transfer vessels
access in and around the vessel
Our preferred way for you to send your completed MSF5100 to us is by email to the appropriate Marine Office.
Make a payment
Our preferred payment method for all Survey or Chargeable Inspection applications is credit or debit card via the GOV.UK Pay online payment facility.

Paying for a Survey or Inspection This link should be used for any Survey or Inspection being undertaken by any of our Marine Offices (Aberdeen, Belfast, Cardiff, Colchester, Glasgow, Hull, Liverpool, Plymouth, Southampton, Stability and Plan Approval Unit).
