This toolkit consists of:

  • guidance on disclosure of cervical screening history review results and applying duty of candour
  • guidance on the review and classification of previous screening results
  • a pathway description of the cancer audit process
  • a pathway description of the disclosure offer process
  • a pathway process for cytology slide reviews
  • a pathway process for histology slide reviews
  • a checklist for cervical screening provider lead (CSPL) actions
  • a checklist for disclosing clinician actions
  • patient information on the screening history review process
  • a form for the patient to complete, indicating if they wish to receive the outcome of their review

The patient information leaflet is also available in A4 PDF format to print out and provide for people who cannot access digital information (see list above).

The patient review results response form is provided in PDF format on this page to print out.

An e-learning module on disclosure of audit results and duty of candour is available for health care professionals involved with providing audit results to members of the public.
