Use these forms to either:

• apply to trainer-book theory tests for your pupils

• apply to change your business details or add, change or remove staff members who can book tests

You need to be registered with the Driver and Vehicle Standards Agency (DVSA) as a business to do this.

If you have a business ID from DVSA

1. Download and fill in the form.

2. Save the form on your computer or device.

3. Email the form to ttnationaloperations@dvsa.gov.uk.

If you do not have a business ID from DVSA

Email DVSA to register as a business before you fill in the form.

Use the subject title Register for a business ID to book theory tests for pupils.

You need to include your:

• full name
• address and postcode
• date of birth
• telephone number
• approved driving instructor (ADI) personal reference number or instructor reference number

DVSA will contact you with your business ID when youve been registered. You need to use this to fill in the form.
